FACILITY FAILED TO PROVIDE APPROPRIATE CARE FOR RESIDENTS WHO ARE CONTINENT OR INCONTINENT OF BOWEL/BLADDER, APPROPRIATE CATHETER CARE, AND APPROPRIATE CARE TO PREVENT URINARY TRACT INFECTIONS.

State Findings:

Based on observation, interview, and record review the facility failed to ensure that a resident who is incontinent of bladder receives appropriate treatment and services for four (Resident #7, #10, #8 and #2) of ten residents reviewed for incontinent care, in that:

– CNA A failed to ensure Resident #7 received appropriate care during incontinent care resulting in Resident #7 moaning, screaming and flinching.

-The facility staff failed to provide timely incontinent care to Resident #10. Resident #10 was found screaming complaining of itchiness to her buttocks.

-The facility staff failed to provide timely incontinent care to Resident #8 and her brief was heavily wet with urine and stool.

-The facility staff failed to provide timely incontinent care to Resident #2 and her brief was saturated with urine and stool.

These failures affected 4 residents who required assistance with incontinence care at risk for discomfort, pain, skin breakdown, cross contamination and urinary tract infections.

In an interview on 11/9/2018 at 2:16 p.m. with CNA D, she stated that staff was supposed to checked for incontinent care every 1 to 2 hours. When asked when was the last time Resident #2 was checked for incontinent care, CNA D responded I don’t know the real time because it’s been fast pace today but it was around 11:00 a.m. , but I also did incontinent care at 7:00 a.m. Resident #2’s FM was not there on either times.
