Based on observation, record review, and interview, the facility failed to ensure residents were given physician prescribed medications within the timeframe they were ordered for 3 (Residents #1, #2, #3) of 3 (#1, #2, #3) sample mix residents.

State Findings:

The Department of Health & Human Services conducted an inspection of the facility. The following highlighted decencies listed below were found in a public survey.

Honor the resident’s right to voice grievances without discrimination or reprisal and the facility must establish a grievance policy and make prompt efforts to resolve grievances.

Based on record review, and interview, the facility failed to ensure the Ombudsman and residents with concerns and complaints regarding late medication administration in the facility were allowed to be voiced through grievances as part of the process of Resident Rights for 1 (Resident #1) of 3 (#1, #2, #3) sampled residents. This failed practice had the potential to affect 62 residents who resided in the facility, according to the Resident Census and Conditions of Residents form provided by the Director of Nursing (DON) on 05/22/23.

Provide appropriate treatment and care according to orders, resident’s preferences and goals.
