Based on a review of clinical records and staff interviews, it was determined that the facility failed to provide nursing services consistent with professional standards of practice by failing to follow physician orders for diabetes management for one resident (Resident 7) out of 25 sampled.

State Findings:

The Department of Health & Human Services conducted an inspection of the facility. The following highlighted decencies listed below were found in a public survey.

Provide appropriate treatment and care according to orders, resident’s preferences and goals

Based on a review of clinical records and staff interviews, it was determined that the facility failed to provide nursing services consistent with professional standards of practice by failing to follow physician orders for diabetes management for one resident (Resident 7) out of 25 sampled.

Provide appropriate care for residents who are continent or incontinent of bowel/bladder, appropriate catheter care, and appropriate care to prevent urinary tract infections.

Based on clinical record, select facility policy and incident report reviews and staff interviews it was
determined that the facility failed to provide individualized services necessary to maintain continence and prevent continued decline and assure the resident received the necessary staff assistance to meet the resident’s toileting needs in an attempt to decrease incontinency for one resident out of 25 sampled (Resident 27).
