FACILITY FAILED TO ENSURE THAT A NURSING HOME AREA IS FREE FROM ACCIDENT HAZARDS AND PROVIDES ADEQUATE SUPERVISION TO PREVENT ACCIDENTS.

State Findings:

Complaint # (AR 550) was substantiated, all or in part, with these findings;

Based on observation, record review, and interview, the facility failed to provide supervision to prevent accidents / injuries for 1 (Resident #1) of 3 (Residents #1, #5 and #7) case mix residents who used side rails. The facility failed to identify side rails as a hazard for this resident and failed to reassess the resident, identify entrapment zones, and implement interventions when Resident #1 was found hanging off the bed with her upper body hanging out of the bed and the lower body remaining in the bed to prevent the potential for fall-related or entrapment injuries. These failed practices resulted in Past Noncompliance at the level of Immediate Jeopardy, which caused, or could have caused, serious harm, injury or death for Resident #1 who was entrapped in the bed rail, and expired from asphyxia, and had the potential to cause more than minimal harm to 6 residents who used side rails, according to a list provided by the Administrator on [DATE] at 4:03 p. m.
