Facility failed to ensure services provided met accepted professional standards of clinical practice for medication administration.

State Findings:

Facility has been federally fined twice in past two years. 

Based on observations, record review, and interviews, the facility failed to ensure services provided met accepted professional standards of clinical practice for medication administration.
Specifically, the facility failed to:
-Ensure expired [MEDICATION(S)], [MEDICATION(S)] and [MEDICATION(S)] (ABH) compound was not administered to Resident #106;
-Obtain an order for [MEDICAL RECORD OR PHYSICIAN ORDER] #1; and
-Clarify [MEDICATION(S)] 5% patch (medicated patch with [MEDICATION(S)]) dosage prior to administering for Resident’s #14.

Based on observations and interviews, the facility failed to establish and maintain an infection prevention and control program designed to provide a safe, sanitary environment to help prevent the development and potential transmission of communicable diseases and infections.

Based on observations, record review, and interviews the facility failed to ensure residents received their meals in a timely manner.
Specifically, the facility failed to ensure:
-There were not more than 14 hours between a substantial evening meal and breakfast the following day;
-The residents in the dining room did not have prolonged wait times of 30 minutes or longer for their meal to be served; and
-In-room trays were delivered to all neighborhoods and served to the residents at the regular posted meal times.

Based on observations, record review and interviews, the facility failed to provide outside dental resources for one (#212) resident reviewed for dental services out of 30 sample residents.

Based on interviews and record review the facility failed to honor resident choices for two (#1 and #105) of three reviewed for self-determination, out of 30 sampled residents.
