State Findings:
Based on record review and interviews, the facility failed to provide assistance with activities of daily living (ADLs) to ensure the highest practicable quality of life and care, for one (#18) of three residents reviewed for bathing and grooming out of 18 sample residents.
Specifically, the facility failed to provide regular showers to Residents #18, who required assistance with ADLs.
Resident #18 was interviewed on 1/24/22 at 10:45 a.m. She is observed sitting up in a wheelchair. Her hair was disheveled and greasy. She said she was not receiving her showers as scheduled on Monday, Wednesday and Fridays. She said she needed help with toileting and showers because her legs were not steady. She said staff would tell her they did not have time to help her with showers so she would ask them for towels so she could shower by herself, but staff would tell her she could not shower alone and she would have to wait for staff to help her.
The director of nursing (DON) and nursing home administrator (NHA) were interviewed on 1/24/22 at 5:15 p.m. The DON said she felt Resident #18 was independent with showering, she said she worked the evening shift on 1/19/22 and saw the resident enter the shower room and said staff likely set the resident up for her shower. She had no response to the resident comments above of staff telling her she needed help. They said staff were supposed to document if the resident received a shower (if they were dependent or independent) in the POC electronic system.
The NHA said she was not sure why there was nothing documented in the resident’s record to ensure the resident was bathed, but felt they had an error in the system.
They acknowledge there was nothing in the resident’s record to indicate the resident was independent with bathing/showering and there was nothing documented to ensure she was bathed. They NHA said they would start education with the staff on 1/25/22 to ensure residents received their showers as there was no documentation provided to indicate Resident #18 was independent with bathing.
