State Findings:

Based on observation, interview, and record review, the facility failed to provide necessary treatment and services, based on the comprehensive assessment and consistent with professional standards of practice, to promote healing and prevent development of pressure injuries for 2 of 4 residents reviewed for pressure injury. (Residents #48 and #49)

The facility did not provide repositioning, off-loading and a pressure relieving device for Resident #48 as indicated by his care plan. The resident developed 3 facility acquired stage 3 pressure injuries and 1 stage 2 pressure injury.

The facility did not provide off-loading to Resident #49’s heels. The resident developed a facility acquired deep tissue injury that worsened over time.

This failure could place residents at risk for the development of new or worsening pressure injuries, decreased quality of care, and decreased quality of life.
