FACILITY FAILED TO PROTECT EACH RESIDENT FROM ALL TYPES OF ABUSE SUCH AS PHYSICAL, MENTAL, SEXUAL ABUSE, PHYSICAL PUNISHMENT, AND NEGLECT BY ANYBODY.

State Findings:

Based on interviews and record reviews, the facility failed to ensure residents had the right to be free from neglect for 1 of 6 residents (CR #1) reviewed for neglect in that:

The facility failed to ensure CR #1, who was admitted to the facility post-craniectomy with part of his skull removed, had a baseline care plan to address his health status. They had no interventions in place to prevent injury to his brain.

The facility failed to ensure CR #1 had fall interventions in place to prevent a head injury. CR #1 was found on the floor with his head at the foot of the bed. The resident was placed back in back with a mechanical lift and expired less than 30 minutes later.

An Immediate jeopardy (IJ) was identified on [DATE]. While the IJ was lowered on [DATE], the facility remained out of compliance at a scope of isolated and severity of actual harm that is not immediate jeopardy due to the facility requiring more time to monitor the plan of removal for effectiveness.

This failure placed residents at the facility at risk of neglect leading to injury or death.
