LAKEWOOD, CO- Director of Nursing said facility is admitting new residents even with staffing shortages, oxygen & feeding deficiencies. Feeding tube bag not changed for 2 days, even though facility documented it was. Resident’s oxygen tube not changed for 4 months.
ARVADA, CO- Director admits she did not follow procedures, the facility failed to promptly address and attempt to resolve resident group complaints and grievances concerning issues of resident care and life.
DENVER, CO- Resident did not receive the anticoagulant medication between 3/3/21 (admitted ) and 3/9/21, representing 12 missed doses. Although the medication was not received from the pharmacy until 3/9/21, it was recorded on the MAR as given on several occasions.
COLORADO SPRINGS, CO- Facility failed to ensure two (#12 and #24) out of four sample residents were kept free from abuse. Facility failed to ensure oxygen was administered according to physician orders for Residents #11 and #14.
PUEBLO, CO- Director of Nursing admits “Resident #5 and Resident
#22 were at risk for pressure ulcers and skin breakdown” after not being moved for hours.
