FACILITY FAILED TO PROTECT EACH RESIDENT FROM ALL TYPES OF ABUSE SUCH AS PHYSICAL, MENTAL, SEXUAL ABUSE, PHYSICAL PUNISHMENT, AND NEGLECT BY ANYBODY.

State Findings:

Based on observation, interview, and record review, the facility failed to ensure the right to be free from neglect was provided for 1 of 29 residents reviewed for neglect. (Resident #81)

The facility did not provide CPR to Resident #81 who was a full code. The resident became unresponsive and was drooling while at the nursing station. Resident #81 was taken to her room and transferred to her bed, still not responsive to verbal or tactile stimuli. Two RNs did not verify the resident’s code status and did not provide CPR. Resident #81 was pronounced dead at the facility.

An Immediate Jeopardy was identified on [DATE] at 8:56 a.m. The Immediate Jeopardy (IJ) was removed on [DATE] at 2 p.m.; however, the facility remained out of at a pattern of potential for more than minimal harm due to the facility’s need to complete in-service training and evaluate the effectiveness of the corrective systems.
