FACILITY FAILED TO ENSURE THAT A NURSING HOME AREA IS FREE FROM ACCIDENT HAZARDS AND PROVIDES ADEQUATE SUPERVISION TO PREVENT ACCIDENTS.

State Findings:

Based on interview, record review, and review of facility Policy, it was determined the facility failed to ensure each resident received adequate supervision to prevent accidents for one (1) of seven (7) sampled residents (Resident #4).

The facility admitted Resident #4 on 03/21/19 and Occupational Therapy assessed the resident on 03/22/19 to require maximum assist (at least assist of two (2) staff) for transfers and total assist for toileting. In addition, on 03/24/19, Physical Therapy assessed the resident as requiring maximum assistance for transfers. However, according to Certified Nursing Aide (CNA) #2’s documented interview obtained on 03/27/19, and the facility Investigation, undated, on 03/27/19, CNA #2 independently assisted the resident on to the commode and helped wash up the resident, then the resident got up using the grab bars and was standing up for three to four (3-4) minutes. When CNA #2 bent over to pull up Resident #4’s undergarments, the resident sustained [REDACTED].#4 was transferred to the Hospital emergency roiagnom on [DATE], and admitted with a [DIAGNOSES REDACTED].
