State Findings:

Based on interview, record review, review of the facility’s policy and video footage, it was determined the facility failed to have an effective system in place to ensure adequate supervision for five (5) of thirty-two (32) sampled residents (Resident #103, #40, #95, #111 and #133).  

On 06/02/19 at approximately 6:41 PM, Resident #103 exited the facility without staff awareness in his/her wheel chair through Exit Door A when a visitor entered the facility, held the door open and allowed Resident #103 to self-propel through the exit door. The resident self-propelled seven hundred and sixty-seven (767) feet in his/her wheel chair to the back of the facility to a maintenance shed, near exit Door C. At about 6:42 PM, Licensed Practical Nurse #12 walked to Exit Door A and silenced the alarm. However, she did not go outside to check if a resident had exited the facility per the facility policy, instead she returned to her unit.

Interview with Certified Nursing Assistant (CNA) #14, on 06/08/19 at 12:27 PM, revealed she left the facility after her shift which ended around 6:30 PM and went to the employee parking lot where she encountered Resident #103 near a double parking garage. The facility’s failure to provide adequate supervision to ensure resident safety has caused or is likely to cause serious injury, harm, impairment, or death to a resident. Immediate Jeopardy (IJ) was identified on 06/14/19 and was determined to exist on 06/02/19. The facility was notified of the IJ on 06/14/19.
