FACILITY FAILED TO PROTECT EACH RESIDENT FROM ALL TYPES OF ABUSE SUCH AS PHYSICAL, MENTAL, SEXUAL ABUSE, PHYSICAL PUNISHMENT, AND NEGLECT BY ANYBODY.

Based on observation, interview, and record review, the facility failed to ensure each resident was free from abuse and neglect for 10 of 34 residents (CR #3, Resident #18, #27, #35, #32, #8, #14, #17, #15, and #43) reviewed for abuse and neglect.

The facility failed to protect residents from CR #3 who was physically aggressive and abusive toward residents.

The facility failed to implement effective interventions to protect residents from CR #3’s physical and verbal abuse.

The facility failed to ensure residents were free to move around the facility as desired without fear of physical attacks from CR#3.

The facility failed to ensure staff were trained to address and deescalate CR#3 violent and aggressive behaviors.

The facility retained CR#3 whose needs could not be met by the facility.

These failures affected 10 residents who were placed at risk of abuse and neglect.
