FACILITY FAILED TO PROTECT EACH RESIDENT FROM ALL TYPES OF ABUSE SUCH AS PHYSICAL, MENTAL, SEXUAL ABUSE, PHYSICAL PUNISHMENT, AND NEGLECT BY ANYBODY.

State Findings:

Based on review of clinical records and information provided by the facility, as well as staff interviews, it was determined that the facility failed to ensure that residents were free from neglect for one of one resident reviewed (Resident 1), resulting in a fractured ankle for the resident.

Information submitted by the facility, dated (MONTH) 21, 2019, revealed that Nurse Aide 1 attempted to transfer Resident 1 independently, and the resident’s transfer status was that she needed the assistance of two, which was on the resident’s care plan, in Point of Care (electronic medical record system that is accessible to nursing staff), and on the resident’s focus sheet. The information indicated that Nurse Aide 1 did not check this information.

A written statement completed by Nurse Aide 1, dated (MONTH) 20, 2019, indicated that Resident 1 rang her call bell and stated that she had to go to the bathroom. She did not check the focus sheet for Resident 1 and she thought she could transfer the resident herself, but the resident fell.
