Facility failed to honor the resident’s right to a safe, clean, comfortable and homelike environment,
including but not limited to receiving treatment and supports for daily living safely.

State Findings:

Based on observation, interview, and record review, the facility failed to provide a safe, clean, comfortable, and homelike environment, including but not limited to receiving treatment and supports for daily living safely, to include maintenance services necessary to maintain a sanitary, orderly, and comfortable interior on 2 of 3 halls (100 and 200), the lounge area, at the nurses’ station, dining room, and in the TV viewing area in that:

-The facility failed to immediately move residents out of rooms that had visible sewage on the floors, inoperable PTAC heating units and mold on the hallway. Residents #1, #5, #7 and #18 were residing on hall 200 in rooms with standing water, strong sewage odors, unflushable toilets full of waste, and non-working lights.

-The facility failed to provide residents on the Memory Care Unit with a secure door that was operable which placed them at risk of elopement.

-The facility failed to provide residents with a sanitary environment free of sewage backflow into resident occupied areas.

-The facility failed to provide residents with working toilets, a secure, non-leaking roof, and a pest free environment.

-The facility failed to provide residents with a sanitary environment. The facility did not put interventions in place to prevent residents from propelling through sewage and having no means to sanitize their hands or wheelchair wheels.

-The facility failed to provide residents with a mold-free environment after visible evidence of a black colored, mold-like substance appeared after unrepaired roof leakage occurred during Hurricane Harvey.

-The facility failed to provide residents with a functioning heating system when the outside temperature ranged from 37 degrees Fahrenheit to 72 degrees Fahrenheit.

-The facility failed to provide residents with a safe living environment by failing to secure the 200 hall wing damaged in 8/2017.

These failures affected 61 residents placing them at risk of re-circulating contaminants throughout the facility which could result in a diminished quality of life, illness, infection, injury, and hospitalization .
