FACILITY FAILED TO GIVE RESIDENTS PROPER TREATMENT TO PREVENT NEW BED (PRESSURE) SORES OR HEAL EXISTING BED SORES.

LEVEL OF HARM –ACTUAL HARM

Complaint # (AR 766) was substantiated (all or in part) with these findings:

Based on observation, record review and interview, the facility failed to ensure necessary treatment and services were provided to promote the healing of pressure ulcers for Resident #3. The facility failed to ensure the physician was promptly consulted regarding deterioration of pressure ulcers and need for treatment changes for 1 of 1 (Resident #3) case mix resident who had a pressure ulcer. The failed practice resulted in a pattern of harm for Resident #3 whose pressure ulcers continued to deteriorate and had the potential to affect 6 residents in the facility who had a pressure ulcer.

On 11/3/15 at 9:45 a.m., Licensed Practical Nurse (LPN) #1 provided wound care for the resident. A large dressing was removed from the coccyx area to reveal a pressure ulcer that measured 9.0 cm x 4.8 cm x 1.8 cm. There was a small area of white slough in the base of the wound with 1.7 cm undermining at 6 o’clock. LPN #1 stated, she had a deep tissue injury (DTI) when she returned from the hospital, but it actually got to looking better. Then it quickly deteriorated and just sort of fell out. I was shocked. It happened so fast I thought maybe it was a Kennedy ulcer. I thought she maybe needed to go to the wound clinic, but she’s on Hospice and they don’t allow that. The wound was cleaned with Sea Cleanse and 4x4s. Gauze soaked with 1/2 Dakins was packed into the wound and then covered with a large [MEDICATION NAME] dressing.

A pressure relieving boot was removed from the resident’s right foot. A dressing to the right outer ankle was removed and revealed a pressure ulcer that measured 2.7 cm x 2.1 cm. The wound bed was yellow slough and the peri wound was bright red. The wound was cleaned with Sea Cleanse and 4x4s. Silver Alginate was placed in the wound bed and it was covered with a dressing. LPN #1 stated, she’s on Hospice so I can’t be real aggressive on treating these.

On 11/3/15 at 4:00 p.m., the Director of Nursing (DON) was asked if Resident #3 had gone to the wound clinic as it was written in the physician progress notes [REDACTED].

On 11/4/15 at 9:25 a.m., LPN #1 was asked why the physician wasn’t notified of the deterioration in the wounds and why the treatments weren’t changed sooner when the wounds failed to improve. He stated, I have learned a lot since yesterday with this. It’s my fault and it won’t happen again.
