State Findings:

Based on observation, interview, and record review, the facility failed to ensure residents received care, consistent with professional standards of practice, to prevent pressure ulcers and ensure the residents did not develop pressure ulcers unless the individual’s clinical condition demonstrated they were unavoidable; and ensure residents with pressure ulcers received the necessary treatment and services, consistent with professional standards of practice, to promote healing and prevent new ulcers from developing for one (Resident #7) of three residents reviewed for pressure ulcers.

The facility failed to provide to repositioning or pressure relieving devices for Resident #7’s chair and as a result, the resident developed facility-acquired, Stage III pressure ulcers.

This failure placed residents at risk of developing facility-acquired pressure ulcers and delay the healing of pressure ulcers.
