FACILITY FAILED TO PROVIDE BASIC LIFE SUPPORT, INCLUDING CPR, PRIOR TO THE ARRIVAL OF EMERGENCY MEDICAL PERSONNEL, SUBJECT TO PHYSICIAN ORDERS AND THE RESIDENT’S ADVANCE DIRECTIVES.

State Findings:

Complaint # (AR 486) was substantiated (all or in part) with these findings.

Based on record review and interview, the facility failed to ensure basic life support measures, including cardiopulmonary resuscitation (CPR) were provided to attempt to revive 1 (Resident #1) of 1 case mix resident who experienced cessation of vital signs and was a full code. This failed practice resulted in past noncompliance at the level of immediate jeopardy, which caused or could have caused serious harm, injury or death to Resident #1, who had a documented full code status and did not receive CPR upon cessation of vital signs, and had the potential to cause more than minimal harm to 36 residents who had physician’s orders [REDACTED]. The Administrator was notified of the Past Immediate Jeopardy on [DATE] at 3:25 p.m.

On [DATE] at 12:25 p.m., the Administrator was asked if LPN #1 was currently CPR certified. The Administrator stated, She was hired on [DATE] .She is currently active with her CPR certification. She just didn’t think she needed to do CPR .

The Facility Investigation Report for Resident Abuse, Neglect, Misappropriation of Property and (&) Exploitation of Residents in Long Term Care Facilities, Division of Medical Services (DMS) form 762 Form documented, .Date and Time of Discovery: [DATE] Time 11:30 AM . The administrator, medical director, and law enforcement were notified. The nurse was suspended on [DATE] pending completion of an investigation. The director of nursing interviewed (LPN #1) on [DATE] regarding her documentation on [DATE] involving the death of (Resident #1). (LPN #1) was not able to explain why CPR was not initiated or EMS (emergency medical services) called after she found her patient unresponsive and without a pulse. It was concluded that she failed to follow the full code physician order [REDACTED]. Staff assigned to the hall on [DATE] were interviewed and with no additional findings noted. An in-service was conducted educating all current nurses and any new hires on resident code status. An audit was conducted on code status orders for all current residents to ensure accuracy with no negative findings. The facility will conduct random mock codes to ensure that staff has the appropriate knowledge on how to check code status and carry out CPR when indicated
