State Findings:

Based on interview and record review, is was determined the facility failed to:
~ perform ongoing thorough skin assessments;
~ provide thorough catheter care;
~ identify a change in condition; and
~ notify the physician when a resident exhibited a change in condition for one (#1) of three sampled residents who were reviewed for catheter care.
The resident was seen in a urology clinic on 02/13/19 and the posterior portion of the resident’s penis was found to be completely eroded (filleted) from the tip of the penis to the base of the penis. It was open with a creamy discharge all over the area.
A hospital Urology clinic note, dated 02/13/19 at 9:30 a.m., documented, .Pt. was here just (MONTH) 27 with Resident as well as I performed UDS test [DATE] penis was fine during that visit pt. stated it has recently middle of (MONTH) to know that this happened. Pt. kept saying I know something is wrong, it’s my body and it’s not supposed to be like this . Pt. arrived for catheter change, he was stating that he (sic) ‘something’ is wrong he is leaking all from the side its ripped the Nurses at Grace Living center tell me its ok it will be fine, (resident #1) was saying , ‘They don’t use the stickers to hold the catheter I want some before I leave. I escorted (resident #1) to the catheter room assisted him on the table and pulled his pants down removed his diaper he was very guarded for his groin area and he stated he was sorry it just hurts really bad, upon my exam it was noted that the posterior part of pt.’s penis is completely eroded (filleted) from the tip of the penis to the base of the penis itis (sic) open and creamy discharge all over the area. Pt stated the bag hangs all the time and pulls and the Nurses at Grace Living Center won’t do anything about it. I told the patient I want to get a provider to look at this I asked (PA’s name deleted) to come see and he in turn went and got (physician’s name deleted) .had agreed to see the patient today and try and schedule him for surgery to get a SPT tube. Patient is in total agreement with plan and ready to move forward to fix this, he is tired of the pain and mess pt. was very emotional.
