FACILITY FAILED TO ENSURE THAT A NURSING HOME AREA IS FREE FROM ACCIDENT HAZARDS AND PROVIDES ADEQUATE SUPERVISION TO PREVENT ACCIDENTS.

State Findings:

Based on observation, interview, record review, and review of the facility’s policy, it was determined the facility failed to ensure one (1) of six (6) sampled residents received adequate supervision and assistance to prevent accidents. On 12/19/2019 between approximately 12:30 PM and 1:00 PM, Resident #1, who the facility had assessed to be at risk for elopement, cut off his/her Wanderguard bracelet, entered the exit code to the front door of the facility, and exited the facility without staff knowledge. Resident #1 crossed the main road in front of the facility and walked approximately 300 feet along the side of the road and was approximately four (4) four to five (5) houses down from the facility before staff reached the resident and assisted him/her back into the facility. However, the facility failed to assess the resident for injury after returning to the facility, failed to investigate the incident, and failed to document the incident in Resident #1’s medical record.
