State Findings:

Based on observation, interview, record review, the facility failed to provide each resident the necessary behavioral health care and services to attain or maintain their highest practicable mental and psychosocial well-being for one (Resident #44) of four residents reviewed for behavioral health services.

The facility failed to timely provide Resident #44 with behavioral health services to ensure psychosocial well-being.

This failure could place residents at risk for their mental and psychosocial needs not being met.

Review of Resident #44’s clinical records revealed the MD had never conducted a face to face visit and assessment of the resident since his admission to the facility. Resident #44 had only been seen by the NP on 11/13/18, 12/13/18 and 01/18/19. Observation and interview of Resident #44 occurred on 01/15/19 at 7:34 AM in the secured unit. He sat on his bed in his room and when asked how he was doing, he responded, I’ve lost my mind. I’m living my own hell. He said he had been placed at the facility several months prior from a behavioral hospital. He also said he was depressed, but the facility social worker came by sometimes for a few minutes and would talk to him about moving outside of the secured unit. He said he struggled with sleeping and got about three to four hours of sleep a night, partly due to his mental issues and partly because of staff and residents in the secured unit interrupting his sleep by wandering into his room or just being noisy. He said he felt lonely and said there were no residents on the secured unit he could talk to and socialize with.

Further interview with Resident #44 on 01/15/19 at 9:33 AM revealed he felt he was getting more and more depressed. He said a doctor came in and asked him one or two questions real quick and felt the doctor did not really care what he had to say. He said he was trying to do the right thing and staff were nice to a point, but they don’t want me to talk about how depressed I am.
