FACILITY FAILED TO ENSURE THAT A NURSING HOME AREA IS FREE FROM ACCIDENT HAZARDS AND PROVIDES ADEQUATE SUPERVISION TO PREVENT ACCIDENTS.

State Findings:

Based on clinical record review, review of select facility documentation, as well as staff interview, it was determined that the facility failed to implement interventions to prevent an accident during ambulation, resulting in harm from a fall with a facial laceration requiring the resident being sent to the hospital and receiving sutures (row of stitches holding together the edges of a wound) for one of three residents reviewed (Resident 1).

Review of Restorative Nursing Status assessment dated (MONTH) 5, 2019, revealed Resident 1 has fluctuating ADL’s (activities of daily living), poor safety awareness, and a need for restorative care (program available in nursing homes that helps residents maintain any progress they’ve made during therapy treatments) for ambulation with a plan to ambulate to and from

the dining room with gait belt, hand-held assistance, and shoes or non-skid socks.

Review of facility’s incident report dated (MONTH) 17, 2019, revealed that Resident 1 experienced a fall on this date. Further review of the incident report revealed Nursing Assistant (NA) 2 was walking resident from the dining room to her bedroom after dinner. NA 2 needed to quickly assist another resident and Resident 1 lost her balance, fell to the floor onto her buttocks, then fell back and hit the back of her head on the floor. Review of the witness statement attached to the incident report revealed that NA 2 let go of Resident 1 to redirect two other residents and Resident 1 fell to the floor.

During an interview with NA 2 on (MONTH) 30, 2019, at 1:39 PM she revealed she could not remember much about the incident but recalled that she was attempting to swish another resident out of a third resident’s room and let go of Resident 1 in doing so.

During an interview with the Nursing Home Administrator on (MONTH) 30, 2019, at 1:15 PM, she revealed that NA 2 made the wrong choice to let of Resident 1 but should have instead ensured her safety before intervening with the other residents.
