State Findings:

Based on review of facility policy, clinical record review, review of facility documentation, and interviews with residents and staff, it was determined that the facility failed to ensure residents were free from neglect during a transfer via a mechanical lift resulting in actual harm to Resident R1 sustaining fractures of the left leg distal tibia and fibula and Resident CL2 sustaining a laceration on the right foot for two of four residents reviewed (Residents R1 and CL2).

Interview with the Nursing Home Administrator on (MONTH) 20, (YEAR), at 2:30 p.m. confirmed that transfers were not completed appropriately for Residents R1 and CL1.

The facility failed to ensure that transfers using the assistance of two staff were completed resulting in harm to Residents R1 and CL1.
