State Findings:
Based on observations, interviews, and record reviews, it was determined the facility failed to ensure residents were not neglected and care was provided timely for three (#3, 9 and #10) of six sampled residents reviewed for neglect.
For resident #3, there was a delayed notification of the physician when the resident’s condition worsened. The physician ordered antidiarrheal medication, which was not administered as ordered and the resident’s skin started breaking down. The physician ordered labs, which were never obtained. The resident’s condition worsened and the physician ordered a diuretic medication, which took over 7 hours to administer. Also, records documented a lack of fluid intake monitoring during the time the resident experienced loose stools. On [DATE] at 12:58 a.m. the resident expired in the facility. The DON (director of nurses) identified 106 residents resided in the facility.
On 08/07/19 at 8:09 a.m., the resident was observed sitting in his bed with approximately three inches of loose brown liquid stool enclosed between his upper thighs. The room smelled strongly of bowel movement, and the top sheet covering the resident had an approximately three inch watery brown stain above the resident’s upper thighs. The resident’s fingernails were noted to have about two millimeters of a brown substance under each fingernail.
At 8:48 a.m., CNA #3 (certified nurse aide) entered the room and said, You are covered (with bowel movement). She stated the resident was completely soaked and had not been changed all night. The resident said, I’ve been laying in pee and (explicit term for bowel movement) all night. CNA #3 was asked what made her think the resident had not been changed during the night. She stated it was not the first time the resident had been found in this condition after the night shift. The resident stated he told the administrator he was not being provided incontinent care at night and he was often left sitting in liquid stool, but nothing gets done.
