Facility failed to ensure one (1) of three (3) sampled residents (Resident #1) was free from resident to resident physical abuse. On 06/26/2020, staff discovered Resident #1 slumped over in bed bleeding with a one (1) centimeter laceration to the head.

State Findings:

Based on observation, interview, record review, and facility policy review it was determined the facility failed to ensure one (1) of three (3) sampled residents (Resident #1) was free from resident to resident physical abuse. On 06/26/2020, staff discovered Resident #1 slumped over in bed bleeding with a one (1) centimeter laceration to the head.

Review of the Nursing Progress Notes, dated 06/26/2020, revealed staff discovered Resident #1 in bed with the top of his/her head covered in blood and blood running down his/her face. The note revealed the nurse observed a laceration to the top left side of the resident’s head, a small laceration beneath the right eye, a large bruise with minor swelling to the left side of the resident’s neck extending up behind the left ear, a bruise to the top of the right thigh, and a skin tear to the top of the right forearm. Further review revealed the facility transferred Resident #1 to the hospital for further evaluation.

Interview with Patient Care Attendant (PCA) #3, on 07/14/2020 at 11:29 AM, revealed when she answered Resident #2’s call light (who shared a room with Resident #1) and entered the room she saw Resident #1 was a bloody mess. She stated she asked the resident what happened, but he/she just looked at her and would not say anything. According to PCA #3, the privacy curtain between the beds was open and Resident #2 was leaning over in bed, and his/her reacher device was lying on the floor between the beds

Further interview with LPN #1 revealed she and PCA #4 checked on Resident #1 the following day and asked the resident how he/she got the knot on his/her head. According to LPN #1, the resident stated another resident hit him/her on the head with those things that extend you can grab things with.
