State Findings:

Based on observation, interview, record review, and review of the facility’s policies, it was determined the facility failed to have an effective system in place to ensure adequate monitoring of pressure ulcers for seven (7) of nine (9) sampled residents (Residents #1, #7, #13, #48, #54, #57, and #67). Skin assessments were not completed weekly per facility policy to ensure the identification of new pressure ulcers. In addition, the facility failed to systematically investigate and analyze to determine underlying causes, which led to facility acquired pressure ulcers (new and worsened).

The facility failed to identify and prevent new pressure ulcers for Residents #1, #13, #54, and #67. In addition, the facility failed to follow physician’s orders [REDACTED].#7, #48, and #57.

The facility’s failure to provide the necessary treatment and services, consistent with the professional standards of practice, to promote healing, prevent infection and prevent new pressure ulcers from developing has caused or is likely to cause serious injury, harm, impairment, or death to a resident. Immediate Jeopardy was identified on 04/10/19 and the facility was notified of the Immediate Jeopardy on 04/10/19.
