FACILITY FAILED TO ENSURE THAT A NURSING HOME AREA IS FREE FROM ACCIDENT HAZARDS AND PROVIDES ADEQUATE SUPERVISION TO PREVENT ACCIDENTS.

State Findings:

Based on interviews, review of the manufacturer’s information, the facility’s policies titled Vehicle Safety Program and Abuse Prevention Program, a complaint received by the Alabama State Survey Agency, Resident Identifier (RI) #1’s medical record, the facility’s investigation file, RI #1’s Emergency Medical Service (EMS) run report and the local hospital’s medical record, the facility failed to ensure Employee Identifier (EI) #3, the Activity Director, who transported RI #1 on 8/1/2019, properly secured the resident in the facility’s van prior to transport.

On 8/1/2019, EI #3 failed to secure RI #1’s lap/seat belt before she proceeded to transport the resident to [MEDICAL TREATMENT]. When the vehicle made an abrupt stop, RI #1, who was not wearing a lap/seat belt, fell from the wheelchair and hit his/her head. EMS was called, and they transported RI #1 to the local hospital, where the resident was diagnosed as having a medial distal femur fracture.

This deficient practice placed RI #1, one of three sampled residents reviewed for transport needs, in immediate jeopardy, as it was likely to result in serious injury, harm, impairment or death.
