FACILITY FAILED TO PROVIDE APPROPRIATE PRESSURE ULCER CARE AND PREVENT NEW ULCERS FROM DEVELOPING.

State Findings:

Based on record review and interview, the facility failed to provide care and services to prevent the development of an avoidable pressure injury, for 1 (R #1) of 4 (R #s 1, 2, 3 and 4) residents that were reviewed for pressure ulcers. This deficient practice caused harm to R #1 by creating a Deep Tissue Injury (DTI) due to facility staff not monitoring the residents air mattress.

On 08/19/19 at 3:42 pm, during an interview, LPN (Licensed Practical Nurse) #1 stated R #1’s wound was a deep tissue injury on 06/03/19. She stated it was discovered when she turned her (R #1) on 06/03/19 and a blackened area had spread over her buttock area. The LPN stated she discovered this doing wound care on her other re-opened pressure wound. The LPN stated the bed malfunction from the afternoon of Sunday (06/02/19 until Monday morning (06/03/19) contributed to the DTI. She stated the Promat mattress should have been checked to make sure it was plugged in and turned on. She also stated it was off for about a day. The LPN stated the Certified Nursing Assistants (CNAs) and nurses are supposed to be making sure the machine is functioning and working. The LPN stated the mattress moving up and down possibly bumped the cord off the wall.
