State Findings:

Based on interview and record review the facility failed to provide quality care for 1 (R #10) of 3 (R #s 10, 11 and 12) residents looked at for pressure sores, change in condition, physician orders and appointments. These deficient practices caused R #10’s medical condition to continue to worsen without staff identifying signs that the resident was having a change in condition that eventually ended with R #10 being admitted to the hospital.

On 10/16/19 at 7:53 AM, during an interview with the Unit Manager/Wound Care Nurse, she stated that R #10 came in with her unstageable pressure ulcer to her right heel. She stated that the cap to the right heel was all eschar and that you don’t have any idea what is under the eschar, or how deep the wound might be. She stated that because of the odor she had spoken to the Nurse Practionier (NP) about it. Because of the eschar the NP didn’t want to have a culture done on it, so the NP wanted staff to pay attention to other signs and symptoms that the wound could be getting infected. The Wound Care Nurse/Unit Manager stated that at this time she was doing wound care and the wound was slowly getting better. She confirmed that if R #10 was having purulent drainage as noted on the SIRS 08/12/19 and 08/19/19, that was a sign that her wound was likely infected and that identifying this sign was missed by staff. She stated that she put the nurses in charge of doing the wound care while she was gone, and they would be following all physician’s orders for resident wounds.

On 10/16/19 at 11:35 AM, during an interview with the Director of Nursing (DON), she stated that all labs that residents have done are automatically uploaded into the system. So if there is a lab that is not in R #10’s chart it wasn’t done. The DON stated that the previous DON had left a total mess of paper documents and no indication of what had been scanned into the residents file and what had not been scanned in yet. The DON also stated that the Unit Manager in charge of the hall that this resident was on at that time quit and walked out the day she was hired to be the DON in the facility.
