FACILITY FAILED TO ENSURE THAT A NURSING HOME AREA IS FREE FROM ACCIDENT HAZARDS AND PROVIDES ADEQUATE SUPERVISION TO PREVENT ACCIDENTS

State Findings:

Based on record review and interview the facility failed to adequately supervise residents to prevent injuries for 2 (R #21 and #81) of 4 (R #21, #76, #81 and #99) residents reviewed for accidents. Resident #21 was noted to have an injury of unknown origin (broken arm) and R #81 had repeated falls with one resulting in a head injury that required stitches. If residents are not adequately supervised residents have the potential to suffer further injuries. The findings are: Resident #21

Record review of a emergency department physician note dated 03/30/19 for R #21 revealed, . Patient brought to the emergency room via emergency medical service from nursing home with complaints of ecchymosis (a bruise) to the left upper arm.Disposition: 03/31/19 – discharged to Nursing Home. Impression: other displaced fracture of upper end of left humerus (long bone in the arm that runs from the shoulder to the elbow). She needs a follow up appointment with . Humerus treated with immobilization.
