FACILITY FAILED TO PROVIDE APPROPRIATE TREATMENT AND CARE ACCORDING TO ORDERS, RESIDENT’S PREFERENCES AND GOALS.

State Findings:

Based on observation, interview and record review the facility failed to ensure that residents received treatment and care in accordance with professional standards of practice, the comprehensive person-centered care plan, and the residents’ choices for three (Resident #59, #85, and #497) of five residents reviewed for quality of care.

The facility failed to ensure Residents #59, #85, and #497 received treatment and care for their Central Venous Catheters, including routinely monitoring and assessing the site for infections or complications, changing the dressing and access port routinely and as needed, maintenance and monitoring of the port’s patency, and physician’s orders to establish management of the catheter. Resident #59 went without treatment for [REDACTED].#85 for over 20 days, and Resident #497 for over 10 days.

An Immediate Jeopardy (IJ) was identified on 10/16/19 at 2:55 PM. While the IJ was removed on 10/18/19 at 3:12 P.M., the facility remained out of compliance at the severity level of potential harm that is not immediate jeopardy and at a scope of pattern because the facility was still monitoring the Plan of Removal. The IJ template was given to the facility on [DATE] at 2:55 PM.

These failures could place the resident at risk for developing systemic infection, complications with blockage or restrictive access, dislodgement, collapsed lung, and [MEDICAL CONDITION].

In an interview on 10/15/19 at 2:36 PM the NP revealed she was not aware Resident #59, #85, and #497 did not have physician orders for the care and treatment of [REDACTED]. She stated the CVC line needed to be flushed before and after medication administration or every shift if the resident was not receiving medication through the line. The NP stated the dressing should be changed according to the facilities protocol or policy. She stated there were no standing orders for the facility for care and treatment of [REDACTED].
