FACILITY FAILED TO PROTECT EACH RESIDENT FROM ALL TYPES OF ABUSE SUCH AS PHYSICAL, MENTAL, SEXUAL ABUSE, PHYSICAL PUNISHMENT, AND NEGLECT BY ANYBODY.

State Findings:

Based on observation, interview, and record review the facility failed to provide the right to be free from neglect to one of two residents with a [MEDICAL CONDITION] (Resident #71) who were reviewed for neglect.

-The facility failed to have a system in place to train and supervise residents who chose to provide self-suctioning and care to their [MEDICAL CONDITION]. Resident #71 did not receive training or supervision from the Respiratory Therapist or Nursing staff when he performed suctioning to his [MEDICAL CONDITION]. Resident #71 developed a respiratory infection which

required antibiotic therapy.

-The facility failed to have a system in place that provided a resident, with a [MEDICAL CONDITION], emergency supplies/equipment at the bedside. Resident #71 was not provided an ambu bag or replacement inner cannula for his [MEDICAL CONDITION] in case of a respiratory emergency.

Interview on 6/20/19 at 1:24 p.m. with RN L, she [MEDICAL CONDITION] had to have an ambu bag in the room in case of an emergency such as respiratory distress. She said Resident #71 did not have an ambu bag in his room on Monday (6/17/19) and she could not find one on the crash cart. She said she had to page the Respiratory Therapist who found two ambu bags in the supply room. She also said that on Monday (6/17/19) there was a malfunction with Resident #71’s suctioning machine. She said everything was set up, but it would not suction. She said they fixed the machine the dame day due to the survyeor intervention.
