FACILITY FAILED TO PROTECT EACH RESIDENT FROM ALL TYPES OF ABUSE SUCH AS PHYSICAL, MENTAL, SEXUAL ABUSE, PHYSICAL PUNISHMENT, AND NEGLECT BY ANYBODY.

State Findings:

Based on observation, interview, and record review, the facility failed to ensure residents were free from neglect for three (Resident #49, Resident #126, and Resident #174) of 18 residents reviewed for labs.

1.The facility failed to have a system in place to ensure nurses were trained and knew how often to check labs. The facility failed to check the laboratory system containing Resident #49’s UA results from [DATE] to [DATE] and report the results to the physician. The resident did not receive the first dose of antibiotic until [DATE]. As a result, the resident’s health deteriorated due to an unidentified and untreated infection. The resident was admitted to the hospital with [REDACTED]. Resident #49 died at the hospital on [DATE].

2.The facility failed to check the laboratory system containing Resident #126’s lab results from [DATE] to [DATE]. Due to the delay, the physician was not notified and did not order necessary interventions until [DATE].

3.The facility failed to check the laboratory system containing Resident #174’s laboratory results and report the results to the physician from [DATE] to [DATE]. Due to the delay, the physician was not notified and did not order necessary interventions until [DATE].

An Immediate Jeopardy (IJ) identified on [DATE]. While the IJ was removed on [DATE], the facility remained out of compliance at a severity level of actual harm that is not immediate jeopardy, and a scope of isolated because the facility had not had the opportunity to monitor the effectiveness of the Plan of Removal.

These failures could affect residents by placing them at risk for untreated illnesses, and delays in necessary care and deterioration in condition.
