FACILITY FAILED TO PROTECT EACH RESIDENT FROM ALL TYPES OF ABUSE SUCH AS PHYSICAL, MENTAL, SEXUAL ABUSE, PHYSICAL PUNISHMENT, AND NEGLECT BY ANYBODY.

State Findings:

Based on observations, interviews and record review, the facility failed to ensure that residents were free from neglect and received adequate supervision and assistance to prevent accidents for 1 of 1 residents (#10) reviewed for neglect and accidents in that:

1. Resident #10 was not provided supervision while on the memory unit and exited the unit to the fenced backyard unattended. The resident fell and sustained second [MEDICAL CONDITION] her legs and thighs from lying on hot rocks for approximately 1.5 hours. When found, Resident #10 had a body temperature of 107.5, was unresponsive and required immediate hospitalization . The resident died because of heat stroke.

These failures resulted in an Immediate Jeopardy (IJ) situation identified on [DATE]. While the IJ was removed on [DATE], the facility remained out of compliance at s severity level of actual harm with a scope identified as isolated until the secure unit was made safe and the entire staff was in-serviced.

These deficient practices could affect 33 residents who were ambulatory and able to wander through the exit doors of the memory unit into the outside gardens and who were at risk for falls, wandering and elopement due to cognitive deficits, lack of coordination, and unsteady gait and could result in deterioration in health, heat stroke and death.
