FACILITY FAILED TO PROTECT EACH RESIDENT FROM ALL TYPES OF ABUSE SUCH AS PHYSICAL, MENTAL, SEXUAL ABUSE, PHYSICAL PUNISHMENT, AND NEGLECT BY ANYBODY.

State Findings:

Based on observation, interview, and record review , the facility failed to ensure each resident had the right to be free from neglect for one of seven residents (CR#1) reviewed for abuse and neglect.

The facility failed to ensure that CR#1’s call for help for shortness of breath was answered in a timely manner.

The facility failed to ensure that CR#1’s roommate’s (Resident #2) call for staff to help CR#1 was not answered in a timely manner. CR#1 who had shortness of breath and chest pain asked his Roommate (Resident #2) to call 911 (Emergency Medical Services) when staff did not respond to his call light for 30 minutes. EMS responded to call made by CR #1’s roommate (Resident #2) and transferred CR #1 to the hospital where he expired a few hours after he was admitted to the ER.

These failures affected one closed record who died.
