Every month from September 2016 to March 2018, COVINGTON COURT HEALTH AND REHABILITATION CENTER received an OVERALL rating of 1-star out of 5 from CMS (Centers for Medicare and Medicaid Services), the lowest rating possible for a nursing home.

Every month from September 2016 to March 2018, COVINGTON COURT HEALTH AND REHABILITATION CENTER received a HEALTH INSPECTION rating of 1-star out of 5 from CMS (Centers for Medicare and Medicaid Services), the lowest rating possible for a nursing home.

Every nursing home in the United States that participates in Medicare/Medicaid is required to be inspected by state surveyors approximately once a year.  Any deficiencies found are reported, and the nursing home is required to submit a written Plan of Correction to the government, explaining what steps the facility has implemented in order to correct the deficiency and to prevent it from reoccurring.

From May 11, 2012 to November 11, 2017, COVINGTON COURT HEALTH AND REHABILITATION CENTER was cited a total of 7 times for the same deficiency—failing to provide necessary care and service to maintain the highest well being of each resident (F-Tag 309).   In other words, COVINGTON COURT HEALTH AND REHABILITATION CENTER had already submitted to the government six (6) written Plans of Correction for the same deficient practice (that should have been corrected) by the time it was cited for the same violation of federal law again on November 11, 2017.  This history raises the questions of just how intent COVINGTON COURT HEALTH AND REHABILITATION CENTER was in correcting known issues regarding providing necessary care and services to residents and how meaningful its express, written assurances have been to the government.
