On June 9, 2017, NORMANDY TERRACE NURSING & REHABILITATION CENTER was cited for IMMEDIATE JEOPARDY causing HARM by failing to prevent maggots from forming on a resident’s peg tube stoma and vaginal region.

On May 13, 2017, NORMANDY TERRACE NURSING & REHABILITATION CENTER was cited for IMMEDIATE JEOPARDY causing HARM by failing to ensure residents were free from neglect and receive assistive devices to prevent accidents, resulting in a resident sustaining an avoidable fall, resulting in bruising, a 1 to 1.5 inch laceration across the bridge of her nose, and a nose fracture with bruising around the eyes.

On June 10, 2016, NORMANDY TERRACE NURSING & REHABILITATION CENTER received a 25-day denial of payment from Medicare for violations of federal law.

Every month from November 2017 to January 2018, NORMANDY TERRACE NURSING & REHABILITATION CENTER received a HEALTH INSPECTION rating of 1-star out of 5 from CMS (Centers for Medicare and Medicaid Services), the lowest rating possible for a nursing home.

Every month from October 2015 to December 2017, NORMANDY TERRACE NURSING & REHABILITATION CENTER received an OVERALL STAFFING rating of 1-star out of 5 from CMS (Centers for Medicare and Medicaid Services), the lowest rating possible for a nursing home.

Every month from August 2016 to December 2017, NORMANDY TERRACE NURSING & REHABILITATION CENTER received an RN STAFFING rating of 1-star out of 5 from CMS (Centers for Medicare and Medicaid Services), the lowest rating possible for a nursing home.

These low staffing levels significantly reduced NORMANDY TERRACE NURSING & REHABILITATION CENTER’s labor costs.  According to its most recently published Medicare Cost Report, NORMANDY TERRACE NURSING & REHABILITATION CENTER generated annual gross revenues of $12,046,189.
