FACILITY FAILED TO ENSURE THAT A NURSING HOME AREA IS FREE FROM ACCIDENT HAZARDS AND PROVIDES ADEQUATE SUPERVISION TO PREVENT ACCIDENTS.

State Findings:

**NOTE- TERMS IN BRACKETS HAVE BEEN EDITED TO PROTECT CONFIDENTIALITY** Based on observation, interviews, policy review and review of the facility documents, the facility failed to ensure a safe and secure environment related to accidents, which resulted in actual harm to resident (R) C who was being transferred from the bed to the geri-chair by one Certified Nursing Assistant (CNA) rather than two as per the care plan which resulted in a nondisplaced tibia fracture. In addition, resident (R) E was attempting to remove the Continuous Passive Motion (CPM is a postoperative treatment method that is designed to aid recovery after joint surgery) without assistance from staff and her left hand got caught in the machine. This resulted in injury to R E left hand.
