FACILITY FAILED TO ENSURE THAT A NURSING HOME AREA IS FREE FROM ACCIDENT HAZARDS AND PROVIDES ADEQUATE SUPERVISION TO PREVENT ACCIDENTS.

State Findings:

Based on observation, interview, record review, and hospital record review it was determined the facility failed to provide adequate supervision to prevent accidents for one (1) of three (3) sampled residents (Resident #1).

Resident #1 was diagnosed with [REDACTED]. However, the facility failed to ensure the adequate amount of supervision/assistance was provided when they care planned the resident to need one to two staff for incontinent care. On 07/28/18, one (1) CNA provided incontinent care for Resident #1 and when the resident threw his/her good leg (left) to the right side to assist with turning, the resident’s momentum caused the resident to roll off the right side of the bed on to the floor resulting in a fractured right femur. Resident #1 had emergency surgery on 07/29/18.
