State Findings:

Based on observation, interview, and record review, the facility failed to ensure food was prepared in a sanitary and safe environment.

The facility did not prevent the septic system from backing up, causing a foul smelling black substance to come through the drains seeping into the kitchen floor in close proximity to food preparation areas, or prevent rain water from leaking onto a functional light fixture into the food preparation area in the kitchen. The facility continued to serve food to the residents from the kitchen.

The facility did not prevent the grease trap in the courtyard from back up causing a strong odor.

The facility did not have the correct sanitizing chemicals for the dishwasher and three compartment sink.

The facility did not have floors in the kitchen that were not a trip hazard.

This failure could place residents at risk for eating contaminated food and increased fire risk due to electrical problems.

During an interview on 01/31/19 at 10:09 a.m., the maintenance supervisor said the black liquid had been coming up through the kitchen floor drain for a month. He said the black liquid would splatter up through the floor drain and puddle on the kitchen floor around the floor drain. He said anytime the kitchen staff turned on the water in the kitchen sinks, the black sewer water spilled out in the kitchen floor. The maintenance supervisor said the sewer water had a pungent odor. He said they attempted to call several plumbers, but no plumbers would come out to the facility without being paid first due to the facility’s history of non-payment. He said corporate (the owner of the facility) was aware of the black liquid but still had not approved payment to hire a plumber. He said the grease trap had not been pumped due non-payment from the last time the vendor pumped the grease trap. He said the last time the grease trap was pumped was (MONTH) (YEAR). The maintenance supervisor said when it rained, water leaked into the kitchen through the ceiling. He said the rain water seeped through the ceiling, ran down the light fixtures onto food preparation tables and the fryer. He said corporate (the facility owner) was notified and told him to get bids for a new roof. He said he received several bids and sent them to corporate (facility owners) twice, but never heard a response. He said the last time he sent in bids for a new roof was late (MONTH) (YEAR).
