FACILITY FAILED TO PROTECT EACH RESIDENT FROM ALL TYPES OF ABUSE SUCH AS PHYSICAL, MENTAL, SEXUAL ABUSE, PHYSICAL PUNISHMENT, AND NEGLECT BY ANYBODY.

State Findings:

Based on observation, interview, and record review, the facility failed to ensure the rights of residents to be free from neglect for 1 of 3 residents (resident #1) reviewed for hydration, change in condition and abnormal values.

1.The facility failed to monitor Resident #1 fluid intake from [DATE] through [DATE] to ensure she was adequately hydrated. Resident #1 entered the facility at risk for dehydration.

2.The facility failed to consult with the Attending Physician when Resident #1 had a change of condition and was not taking in fluids. The facility failed to reassess interventions for resident #1 regarding weight loss.

3.The facility failed to ensure abnormal lab results received on [DATE] and [DATE] were promptly reported to the physician. Resident #1 was hospitalized on [DATE] with [DIAGNOSES REDACTED]. Resident #1 expired in the hospital on [DATE].

These failures resulted in an identification of an Immediate Jeopardy (IJ) on [DATE]. While the IJ was lowered on [DATE], the facility remained out of compliance at a level of actual harm with a scope identified as a pattern.

This failure affected one resident and placed an additional 2 residents with hydration concerns at risk for dehydration that were on [MEDICAL CONDITION] medications.

In an interview on [DATE] at 5:47 p.m. Hospital Nurse Manager stated that she was told patient was covered in feces and bruises. Stated that there was an APS referral and DR was the admitting physician. Stated that she was told Resident #1 had while yellow plaque, moaning and did not respond to any questions, severally dehydrated. Stated that the nursing home sent resident to the hospital because she was unresponsive. Stated that hospital staff stated that she came for dehydration.

Stated that she worked at a nursing home and that she knows you can’t make people drink but they should have done some kind of intervention. Stated that Resident #1 had abnormal lab values. Stated that resident’s mucus membranes were cracked, skin turgor not good, mouth was inflamed, nutrition not adequate, BUN was 82 and [MEDICATION NAME] was 2.2 meaning resident was in kidney failure.
