FACILITY FAILED TO GIVE OR GET QUALITY LAB SERVICES/TESTS IN A TIMELY MANNER TO MEET THE NEEDS OF RESIDENTS

Based on record review and interview, the facility failed to ensure laboratory services were provided timely to assist the physician in making treatment decisions to ensure the necessary care and services were provided for 2 (Residents #2 and #3) of 8 (Residents #1, #2, #3, #4, #5, #6, #8 and #9) case mix residents who had Physician orders [REDACTED]. The findings are:

Resident #2 had [DIAGNOSES REDACTED]. The Quarterly Minimum Data Set (MDS) with an Assessment Reference Date (ARD) of 3/11/16 documented the resident scored 15 (13-15 indicates cognitively intact) on the Brief Interview for Mental Status (BIMS), and required extensive assistance of two plus people for bed mobility and transfers.

A Physician order [REDACTED].Vitamin D Level Q (every) Yearly (June) every 12 month(s) starting on the 13th for 3 day(s) related to Vitamin D Deficiency, Unspecified

As of 8/3/16 at 9:50 a.m., there was no documentation in the resident’s clinical record of laboratory results for a Vitamin D Level since 12/12/14. At this time, the DON was asked, Can you get me the results of a yearly Vitamin D Level (for Resident #2)? She replied, When we changed to computer system our medical record person put the wrong month in the computer. She stated, Instead of June, it should be December. The DON then handed this surveyor a copy of the Vitamin D Level results for December 2014.

On 8/3/16 at 2:20 p.m., the DON was asked for current Vitamin D level laboratory results for the resident. She stated, We have no idea why the Vitamin D Level was not drawn. Every other lab was drawn but we missed the Vitamin D .I cannot figure out how it happened.

Resident #3 had [DIAGNOSES REDACTED]. The Annual MDS with an ARD of 5/25/16 documented the resident scored 3 (0-7 indicates severely cognitively impaired) on the BIMS, and was totally dependent on the physical assistance of two plus people for transfers and personal hygiene.

As of 8/3/16 at 11:55 a.m., there was no documentation in the resident’s clinical record of laboratory results for a Vitamin D level since 5/29/14.
