Immediately tell the resident, the resident’s doctor and a family member of the
resident of situations (injury/decline/room, etc.) that affect the resident
Based on closed record review, review of a facility self reported incident and investigation documentation and staff interviews, the facility failed to ensure a resident’s family was notified when the resident was identified left on a bed pan for 11 hours, which resulted in a reddened area on the resident’s buttocks in the shape of the bed pan.
Continuing: Review of a facility Self-Reported Incident (SRI) dated 05/08/14 revealed the resident’s family member reported bruises on the resident’s back side. The resident was sent to the hospital on [DATE] and the bruises were discovered there.
