it was determined the facility failed to ensure nurses weekly skin assessments were completed accurately to identify a pressure sore timely, obtain a physician’s orders [REDACTED].#27, 7, 70 and 122) sampled residents with pressure sore included in the complaint and certification survey. The
failure of the nurses to identify Resident #27’s wound before it became unstageable and necrotic, resulted in actual harm. The failure to identify a pressure ulcer before it reached a stage 3, resulted in actual harm for Resident #7. A stage 3 pressure ulcer is full thickness skin loss involving damage or necrosis od subcutaneous tissue that may extend down to, but not through, underlying fascia. The ulcer presents clinically as a deep crater with or without undermining of adjacent tissue. The failure to put preventative measures in place on admission to prevent the development of a pressure ulcer for a resident who was identified as a high risk for pressure ulcers, resulted in actual harm when Resident #70 developed a pressure ulcer.”
A high risk resident is typically someone who is helpless and cannot roll or move on their own. Their care and treatment is documented and they rely solely upon a caregiver to help prevent a pressure sore from developing.
