FACILITY FAILED TO PROVIDE NECESSARY CARE AND SERVICES TO MAINTAIN THE HIGHEST WELL BEING OF EACH RESIDENT

Based on observation, interview and record review, it was determined the facility failed to:
Ensure indwelling urinary catheters were not used without justification for use for one (#6) of six sampled residents with indwelling urinary catheters and failed to ensure the resident received care and services to prevent urinary tract infections. Resident #6 developed seven urinary tract infections from admission on 01/13/15 through 05/13/15. The Resident Census and Condition report identified nine residents with indwelling urinary catheters.

On 05/19/15 at 9:30 a.m., LPN #1 was asked how long the resident had had an indwelling urinary catheter. She stated she was just going to check on it because she had just asked for the catheter to be taken out due to pain. The LPN was asked what the [DIAGNOSES REDACTED] was. She stated she wouldn’t know without looking. She was then asked to review the resident’s chart for the current order. She stated she could not find an order. She was asked if there was an order would it be on the monthly physician orders. She stated, yes. At 9:35 a.m., CNA #3 was asked who was responsible for recording I/O’s. He stated the aides were responsible to collect the readings and they give the information to the nurses. He was asked what is done if an I/O is not submitted at the end of the shift. He stated the nurse will follow up or have the next shift get the reading.

At 10:15 a.m., the resident was asked if staff provided catheter care. She stated, No, and just now I was told if I needed to be cleaned down there to ask. When I was in the hospital they cleaned it every day. She was asked how many urinary tract infections had she had since her admission. She stated, every month since I have been here. The resident was asked if she had to go to the hospital because of any infections. She stated, Yes, I was there for ten days. She was asked what she remembered about her hospitalization. She stated they told me the culture grew out e-coli in my urine and in my blood.
