FACILITY FAILED TO PROVIDE NECESSARY CARE AND SERVICES TO MAINTAIN THE HIGHEST WELL BEING OF EACH RESIDENT

LEVEL OF HARM –ACTUAL HARM

Based on observation, interview and record review the facility failed to provide the necessary care and services to attain or maintain the highest practicable physical, mental, and psychosocial well-being for 4 of 23 residents (CR #16, CR #13 Resident #5 and Resident #21) reviewed for pain management and [MEDICAL CONDITION] care.

CR #16 had a change of condition, she began vomiting on [DATE] during the 2:00pm-10:00pm shift. She was found on [DATE] at 6:50am not responding and not breathing. She was pronounced dead at the hospital at 7:37am.

CR #13 complained of pain for 5 days without receiving pain medication ([MEDICATION NAME]) scheduled or as needed. CR #13 was sent to the hospital at her request and was found to be in [MEDICAL CONDITION] and have a renal mass. She also had a [MEDICAL CONDITION] bag that was not changed and would come off and leak stool all over resident attracting flies.

Staff failed to change [MEDICAL CONDITION] bags as ordered for Residents #5 and #21. This caused the bags to leak stool onto the residents and to cause a foul odor.

This failure affected one resident and placed 42 residents on pain management and four residents that required ostomy care at risk of not receiving assessments, medical care and services, which could have a negative impact on their medical health and well being.
